Punch Card Hold Mailing Form

The annual punch card is not able to be forwarded as it is not first class mail.

Pleae fill out this form if you have your mail forwarded between January and April. Public Works will hold your annual punch card and mail it to you after you return based on the date you provide.

Note that landlords are not able to request an alternate mailing address.
